There is something for everyone!Position Overview:This is a grant-funded position with job responsibilities set forth by grant contract statement of work and related documents. This position will continue as grant funding availability and contract dictate. Community Health Navigator will possess excellent oral and written communication skills, be self-directed, and have a passion to help women, in particular the underserved. The Community Health Navigator will help women enter the healthcare system in relation to breast and cervical cancer screenings. Under minimal supervision, the position provides education in the community about breast, cervical, and colorectal cancer prevention and early detection and assists eligible women in accessing breast and cervical screening services by assessing and reducing barriers. Establishes and maintains community partner relationships and provides outreach and education activities at community events. Position will act as Community Health Navigator 40 hours a week, including evenings and weekends as needed.Essential Functions: The essential job functions align with grant deliverables, and State outreach requirements. These include: 1.Providing education in community about breast, cervical, and colorectal cancer prevention and early detection: a.Small Media b.One-on-one education c.Group education 2.Providing information about resources, including: a.Recommended screening guidelines b.Screening/ health care clinic locations and contact information c.Health insurance enrollment d.Transportation providers e.Dependent care providers 3.Conducting individual assessment of barriers to determine need for direct assistance to access screening with identified population 4.Collecting data 5.Providing direct assistance to reduce barriers such as, but not limited to, the following: a.Educating to address needs (health literacy, fear, how to) b.Providing transportation options (gas cards) terpreting or translating languages d.Scheduling appointment e.Finding a resource for dependent care f.Providing help or referring to health insurance enrollment Other essential functions include: Working independently to develop and maintain the program Coordinating and presenting educational programming throughout the region Helping collect pertinent information for financial reports to State Collecting data, both on individual women and at global level, as required by State of Colorado for grant reporting requirements Working within best practices according to national resources to reach women Maintaining patient confidentiality Attending Targeted Community Outreach assigned trainings Establishing and maintaining relationships with new and existing community partners to include Hospital departments, as well as regional non-profits, clinics, and businesses Receiving and responding to program inquiries and information requests from public Other duties as required to meet grant deliverablesMontrose Memorial +Resources+ Home +Share This Page
