End Date: 05-18-2026 Contract Type: Travel Contract Shift Type: Day
shifts (specific start/end times not provided); On-call rotation
1-3 shifts per week (includes 3 weekends during the 13-week
contract); must be able to respond within 45 minutes when on-call;
holiday availability required. Hours per Week: 32 Candidate Pay

Roles & Responsibilities Perform comprehensive patient assessments
and coordinate individualized hospice care plans Provide expert
pain and symptom management consistent with each patient’s plan of
care Educate and support patients and families on disease
progression, comfort measures, and hospice philosophy Collaborate
closely with physicians, social workers, chaplains, volunteers and
the interdisciplinary team Participate in the on-call rotation and
respond promptly to patient needs when assigned Maintain accurate,
timely clinical documentation in compliance with state, federal,
and organizational policies Deliver compassionate, dignified care
in patients’ homes and serve as a trusted clinical resource for
families Qualifications Current Registered Nurse (RN) license in
Utah or a compact state Minimum of 3 years of nursing experience
(hospice, home health or palliative care experience preferred)
Current BLS certification required Valid driver’s license and
reliable transportation required for home visits Able to respond
within 45 minutes when on call Willingness to work holidays and
participate in weekend coverage as scheduled Strong clinical
assessment, communication, interpersonal and organizational skills
Compassionate demeanor and ability to support patients and families
through end-of-life care Education Active Registered Nurse (RN)
licensure in Utah or compact state Current BLS certification
Knowledge & Skills Patient assessment and care plan coordination
Pain and symptom management in hospice settings Clinical
documentation and use of hospice EMRs (Hospicesoft, Careficient
preferred) Interdisciplinary collaboration with physicians, social
workers, chaplains and volunteers Patient and family education on
end-of-life care and self-care techniques On-call triage and rapid
