La Villa Grande Care Center - Grand Junction, CO Pay Rate -
$40Hr - $50hr DOE BENEFITS OFFERED for FT - Health, Vision, and

with us. FUNCTION: The Physical Therapist will carry out an
individualized program of physical therapy including assessment,
treatment, planning, implementation, education, and communication
to maximize a patient's progress toward achieving functional goals.
The Physical Therapist will also maintain a positive working
relationship with other health professionals and ancillary staff.
QUALIFICATIONS: The requirements listed below are representative of
the knowledge, skill and/or ability required. Minimum
Qualifications: • Knowledge and skills to care for the physical and
developmental needs of the population age groups that will be
receiving services. • Ability to remain calm in emergency or crisis
situations. • Graduate of an American Physical Therapy Association
(APTA) approved physical therapy program. • At least one year of
experience as a physical therapy aide. • Current license from the
state Board of Physical Therapy Examiners. ESSENTIAL FUNCTIONS AND
RESPONSIBILITIES: To perform this job successfully, an individual
must be able to perform each key function satisfactorily.
Responsibilities: - Provides direct patient care, evaluates needs,
consults with specialists, and adjusts care plans. - Designs and
implements patient care based on assessments and involves
significant others. - Conducts therapy within defined standards and
directs support staff. - Communicates effectively to achieve
