HomeCare & Hospice of the Valley is actively searching for experienced, well organized, motivated Home Health nurses.

$4,000 reimbursement relocation costs or sign-on bonus. $2,000 upon hire and completion of orientation and full case load assumed; the remainder paid after successful completion of 1 year of employment (not available to part-time applicants)

Area of Service: Vail/Eagle Valley

Pay Range: $5,500 to $8,320/month

Essential Functions of the Job:
The Home Health Nurse performs initial assessment upon admission to Home Health
Admits patients to Home Health service by accurate entry of patient information, admitting diagnosis and physician's orders
Develops and implements an individualized, realistic, goal and outcome-focused Plan of Care for each patient and updates the Plan of Care per department guidelines. Plan of care relates to admitting diagnosis and/or to changing needs of patient
Case manages patient care when assigned to perform Case Management Duties
Home Health Nurses must participate in Call rotation with one call shift per week and one weekend of call per month

Requirements:
Qualifiications for our Home Health Nurse
The Home Health Nurse must possesses current license to practice as a registered professional nurse in the state of Colorado.
Current BLS verification.
Minimum one year of Home Care nursing experience
One year of acute care experience
Personal vehicle for on-the-job transportation in all road and weather conditions, climate
