Description:HomeCare & Hospice of the Valley is actively
searching for experienced, well organized, motivated Home Health
nurses.$4,000 reimbursement relocation costs or sign-on bonus.
$2,000 upon hire and completion of orientation and full case load
assumed; the remainder paid after successful completion of 1 year
of employment (not available to part-time applicants)What We Offer

$8,320/monthEssential Functions of the Job:The Home Health Nurse
performs initial assessment upon admission to Home HealthAdmits
patients to Home Health service by accurate entry of patient
information, admitting diagnosis and physician's ordersDevelops and
implements an individualized, realistic, goal and outcome-focused
Plan of Care for each patient and updates the Plan of Care per
department guidelines. Plan of care relates to admitting diagnosis
and/or to changing needs of patientCase manages patient care when
assigned to perform Case Management DutiesHome Health Nurses must
participate in Call rotation with one call shift per week and one
