Job Description:

Center for Mental Health, based in Montrose, seeks an Integrated
and Outpatient Therapist for its offices in western Montrose
County, located in Telluride, Naturita and Norwood. This position
also provides services in two local school districts.Incumbent can
expect to work a minimum of one day per week in Telluride, Norwood
and Naturita with the ability to work a flexible or compressed work
schedule to serve client and school needs.*INTEGRATED CARE and
OUTPATIENT THERAPIST - Can be based out of our Telluride, Norwood,
or Naturita Colorado office locations.** Position eligible for our
relocation assistance package up to 5% of accepted annualized base
pay.* Position eligible for a sign on bonus of 5% of accepted
annualized base pay.* Position will receive a 10% geographical pay
differential in addition to offered base compensation.*GENERAL
PURPOSE: *Utilizes a variety of treatment approaches to work with a
diverse client and patient population. Service goals include, but
not limited to, providing prevention, early intervention and
treatment services, as well as, group and family therapy, social
and emotional group activities, skills groups, behavioral supports,
crisis management, care coordination and consultation. Will also
link clients to other services and agencies in the
community.*ESSENTIAL DUTIES AND RESPONSIBILITIES** Delivers and
documents mental health and substance abuse services (including DUI
group facilitation) to individuals, group, family therapy, crisis
intervention and case management services to clients designated by
the Center. These services may be delivered in the office, medical
clinic or within the community.* Conducts mental health status
examinations and diagnostic assessment to establish differential
diagnosis and treatment plans* Meets The Center's standard for:
Client Care hours, Missed Appointments: i.e. DNS, CBT, CBC,
Services Plans, and Completion of All Documentation* May deliver
person centered, culturally sensitive evaluations, trauma informed
services as defined by position. These services may be delivered in
various locations.* Works with client and their systems to identify
treatment goals and objectives.* Maintains professional licensure
and certifications if applicable* Maintains client
charts/documentation according to the standards outlined in the
record keeping procedures, administrative procedures, and quality
assurance procedures of the Center (DAP notes, CCARs, Service
Plans, Diagnosis, etc).* Attends staff meetings, in-service
meetings and peer reviews, supervision sessions, and other meetings
as requested/required* Participates in the design of client
services through needs assessment and discussion with team members
and supervisor* Collaborates with medical personnel and other human
service agencies within the community to coordinate client care
plans* Responsible for making appropriate appointments with medical
and psychiatric staff and collaborates with staff psychiatrists to
coordinate client care plans*Center-Wide Essential Duties and
Responsibilities** Responsible for keeping client information
confidential by adhering to Colorado law and policies and
procedures for The Center.* Responsible for compliance with
applicable federal, state and local laws; professional practice
acts and ethical guidelines; policies and procedures of the Center;
and contractual guidelines.* Responsible for supporting the
Center's Vision, Mission, Values and Beliefs by practicing and
modeling a culture of wellness within a _You Matter _mindset.*
Completes all administrative paperwork and documentation/time sheet
(billing and personnel/payroll) as requested/required* Completes
required competency training in Essential Learning and additional
training as requested or selected by supervisor* Reviews and
acknowledges Center policies and procedures as requested/required*
Works cooperatively and constructively as part of an
interdisciplinary team*Education and Formal Training: ** Master's
level degree in one of the recognized mental health professions
required* Colorado State professional license preferred (LCSW, LPC,
or LMFT); license-eligible required* Experience treating children,
adolescents and families required* Experience in an integrated
setting preferred* Training in conducting mental health status
examinations to establish differential diagnosis and treatment
plans* Experience providing both mental health and substance abuse
treatment in a professional setting*VACCINATION REQUIREMENTS** In
compliance with CDPHE guidelines, all employees at The Center for
Mental Health must provide proof of full COVID-19 vaccination
status. New employees must provide prior to their first day of
work. "Full COVID-19 vaccination status" is defined as having
received the first two doses of Moderna or Pfizer, or one does of
Johnson & Johnson, and at least two weeks have passed since the
second dose. The Center will also provide Medical or Religious

annual flu season (October-December) is highly recommended OR
completion of a Declination must be submitted to Human Resources by
December 31, 2021.Starting salary: $42,947-$51,536 for unlicensed,
master's level; $51,996-$62,539 for licensed, master's level,
commensurate with licensure, certification and relevant experience
