much more. Job Overview: The Patient Navigator Role encompasses
patient navigation for orthopedic patients, pre anesthesia testing
(PAT) for all surgery patients and the ability to staff in Same Day
Surgery as needed. The patient Navigator assists patients through
their pre- and post-experience to ensure the patient understands
what to expect pre and post op, all necessary history and results
of tests are documented in the chart, and care post op is
coordinated. The patient navigator plays an essential role in
facilitating a positive experience and outcome for the patient. The
patient navigator establishes a positive and respectful
relationship with each client. The navigator collaborates with the
inpatient case managers as needed and participates in
multidisciplinary rounds if an orthopedic patient is admitted to
the hospital and facilitates all discharge planning for the
admitted orthopedic patients. The Navigator will also be
cross-trained to the Same Day Surgery RN responsibilities in order
to assist in staffing that department as needs arise. Primary Scope
of Responsibilities: * Works collaboratively with the
anesthesiologist and the patient/family having surgery to ensure
the patient and family are well informed and prepared for surgery
and all documentation is available preoperatively in the patients
chart. This responsibility may include pre-procedure and follow-up
telephonic or electronic check-in, the mailing/shipment of
information or materials, and connecting client to the appropriate
clinical team for instructions, clarifications, or follow-up. *
Works with clinical and administrative teams to schedule patients
for services post operatively. This may include working with
hospital staff, orthopedic physicians, community physical
therapists, and others to determine the best date and time to begin
the services, arrange transportation and travel, transmit
information collected, and seamlessly hand-off any established
client relationship to the appropriate professional. * Keeps an
accurate and complete record of all communications and work
generated. * Maintains cooperative working relationship with
hospital personnel and physicians in order to resolve problems and
to enhance smooth and efficient delivery of services. *
Communicates with physicians, nursing staff, families, and other
disciplines to formulate a plan of care based on patient needs.
Participate in the Transitions of Care team to coordinate the
discharge process as a multidisciplinary plan of care. Provide
frontline leadership support and orthopedic guidance for the
inpatient nursing departments. * Supports the RN Case Management
role with utilization review and discharge planning processes for
orthopedic patients. * Fully functional in the Same Day Surgery RN
role as needed. Population Specific Criteria: * Ensures
developmentally appropriate care for infants, toddlers, children,
adolescents, adults and geriatric patients. Minimum Qualifications:
* 2 years of orthopedic experience required * Excellent customer
service skills required * Same Day Surgery RN experience preferred
* Experience in orthopedic navigation role preferred * Experience
in Pre Anesthesia role preferred * Ability to read and communicate
effectively in English * Bilingual in Spanish highly desired
Education/License/Certification Requirements: * Associate Degree in
Nursing required; BSN preferred * Current Colorado nursing license
(or compact) required upon hire * Current BLS certification
required * Current ACLS certification, or must obtain within 6
months of hire * Current PALS certification, or must obtain within
6 months of hire * Certification in orthopedic nursing recommended
within 2 years of hire *Our hire-in rates of pay are based on years
of relevant experience. Listed below are our hire-in rates for the
position, which includes: minimum pay (no relevant experience) and
mid-point (12 or more years). Minimum = $34.28/hr Mid-Point =
